Community Choices Waiver (CCW)
Through the Office of Aging and Adult Services (OAAS)
Personal Assistance Services inclusive of Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL):
ADLs
- Eating
- Bathing
- Dressing
- Grooming
- Walking
- Using a wheelchair
- Toileting
- Changing positions in bed (bed mobility)
- Moving from one surface to another (transferring)
IADLs
- Preparing & Storing Meals
- Shopping
- Laundry
- Medication reminders
- Medical appointment assistance
- Assistance finding transportation for medical appointments
- Light Housekeeping (to include sweeping, mopping, vacuuming, cleaning of areas used by client ONLY)
Individuals qualifying for this waiver MUST:
- Meet Medicaid eligibility AND…
- Be 21 years old or older AND…
- Meet Nursing Facility Level of Care (LOC)
*While in-home services for Community Choices Waiver and Long-Term Personal Care Services are the same, access to the waiver services provide Support Coordination (case management). Individuals approved for the waiver must be linked to a provider through the assigned Support Coordinator.
Long-Term Personal Care Services
Personal Assistance Services inclusive of Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL):

- Changing positions in bed (bed mobility)

- Light Housekeeping (to include sweeping, mopping, vacuuming, cleaning of areas used by client ONLY)
*Individuals may also receive Long-Term Personal Care Services with approval of the Adult Day Health Care Waiver. These individuals must be linked to a provider through Support Coordination and are required to attend an Adult Day Health Care Center at least 3 days each week, for a minimum of 4 hours each of the 3 days.
Children’s Choice Waiver (CC)

- Meet Louisiana Medicaid eligibility AND…
- Meet the Louisiana definition for developmental disability which manifested prior to age 22 (Revised Statute 25:451.2, Paragraph (11), AND…
- Have an OCDD Statement of Approval
or
- Meet My Place eligibility if age 0-3 AND…
- Meet Intermediate Care Facility- Intellectual Disability (ICF-ID) Level of Care Criteria AND…
- Be 0 through 20 years of age
*Access to services for this waiver provides Support Coordination (case management). Individuals approved for the waiver must be linked to a provider through the assigned Support Coordinator.
New Opportunities Waiver (NOW)

- Meet Intermediate Care Facility- Intellectual Disability (ICF-ID) Level of Care Criteria, AND…
- Be 3 years of age or older, AND…
- Have needs that cannot be met in another OCDD waiver
